What is white tongue coating?

It's a thin film formed when food debris, bacteria, and shed cells build up between the small bumps on the surface of the tongue. It's present in healthy mouths too, and because saliva decreases during sleep, it looks whiter in the morning.

In the clinic, I often hear, “Is it because I'm not taking care of it? Should I clean it more often?” If it's thicker than usual and doesn't come off well with brushing, it may be a sign of a change in the mouth or the body. Just cleaning more often and scraping harder won't break the cycle of the film.

What causes white tongue coating to come back?

The cause is often not just one thing but several overlapping conditions.

The coating builds up thickest on the back third of the tongue. Saliva doesn't reach this area well and the toothbrush reaches it less, so when dry mouth is added, it's the first place to build up again.

Does white tongue coating cause bad breath?

It can. Bacteria in the film break down proteins from food and produce odor compounds, so even after you clean your tongue, your mouth feels stale again after a while.

That said, not all bad breath comes from tongue coating. Cavities, tartar, gum inflammation, and digestive problems also leave odors, and with gum disease the smell lingers. In that case, gum treatment comes before the tongue.

If the smell bothers you, think about when it's worst. If it's worse right after waking up or after not talking for a long time, and it decreases when you drink water or brush, the cause is likely dry mouth and tongue coating. If it stays even after brushing, check the gums or cavities first.

What should you change to reduce white tongue coating?

Reducing the conditions that let it build up again comes before making the tongue clean in one go.

  • When brushing, also gently clean the tongue from the back toward the tip (How to use a tongue cleaner).
  • Remove debris between the teeth with floss or an interdental brush.
  • Drink water often and cut down on the habit of breathing through your mouth.
  • Cut down on smoking and frequent drinking, and get enough sleep.
  • Get regular checkups and scaling.

If you keep scraping the same spot to force it off, you only damage the mucous membrane.

How we look at it at Wonjin Dental

If the tongue stings or burns, the surface turns black or red, the smell remains even after brushing, or there's gum bleeding or toothache at the same time, I don't look only at the tongue. I also check for cavities, the gums, and the possibility of oral candidiasis. I don't just change the tongue cleaner without identifying the cause. If you often have heartburn or acid coming up, it's a good idea to see an internal medicine doctor as well.

What if the tongue coating turns yellow or black?

It can be stained by coffee, smoking, or some medications. If the color changes suddenly or lasts a long time, have it checked. There are also cases, like black hairy tongue where the tongue's bumps grow longer and look black, where the cause needs to be found and managed.

Will gargling with mouthwash get rid of the coating?

It only feels fresh for a moment; the conditions that let the film build up stay the same. Products containing alcohol can make the mouth drier, so if you have dry mouth, alcohol-free products are better.

If a baby has a white film on the tongue, is it tongue coating?

It may be milk residue, but if it doesn't wipe off easily or the baby has trouble feeding, it may be thrush (a Candida infection). Have it checked at a pediatrician or a dentist.

Does white tongue coating mean I have a disease?

In most cases, it's an issue with the environment in the mouth, and it forms in healthy people too. However, if the coating comes with fever, weight loss, or difficulty swallowing, or if it hasn't improved for several weeks, see a doctor to find the cause.
