Why do the front teeth gap again after orthodontics?

The most common cause is a retainer problem. “It's been a few years since my orthodontics finished, but a gap has opened between my front teeth again.” When I see someone like this, the first thing I check is the retainer.

  • The removable retainer wasn't worn enough, or it has worn out or warped and no longer fits
  • The fixed retainer wire behind the front teeth has come loose at one point or broken (including cases where it looks attached but the bond is weak)
  • A habit of pushing the front teeth with the tongue, or biting the lips
  • A bite where the upper and lower teeth overlap deeply, or force concentrates on one tooth
  • Gum inflammation or periodontitis reducing the alveolar bone so the teeth are pushed out

If an adult's front teeth suddenly start to gap, I look at the gums first. If periodontitis is the cause and gum treatment isn't done first, the gap will open again even after it's filled.

When can the gap be filled without redoing orthodontics?

If the alignment and bite are stable, the teeth are no longer moving, and there's no progressing inflammation in the gums, I consider filling it. Small gaps can often be closed in one visit by adding a small amount of front-tooth composite to the sides of both teeth, with almost no trimming.

If you also want to match color and length and the teeth have stopped moving, I consider porcelain veneers. Brinate, placed without trimming, comes in Basic 0.3mm (same day possible), Build-up 0.1mm (about 1 week), and Platinum 0.1mm (about 2 weeks). However, the wider the gap, the more widening the teeth to fill it makes them look broad.

When is orthodontic re-treatment needed?

If the gap keeps widening, the front teeth rotate and push forward, and even the neighboring teeth become disordered, we reposition them before filling. When the retainer doesn't fit or the wire has come off, the bite keeps hitting one spot, or a tongue habit remains, we also stop the cause first. If composite is placed on teeth that are moving, the contacts shift and pieces chip. I don't fill gaps on teeth that are moving. Usually partial orthodontics, moving only a few front teeth, is enough, and redoing the whole arch is rare.

Do I still need a retainer after the gap is filled?

Yes. Composite and porcelain veneers correct shape and gaps; they aren't devices that stop movement. After filling, a new retainer must be fitted or the fixed retainer wire rebonded so the same thing doesn't happen again. Gums don't grow back, so I also don't simply cover up gaps that opened due to alveolar bone loss with composite.

Small gaps are filled; drifting arches are moved. I don't mix the two into one treatment. It's safer to first have it checked to see whether it still fits your teeth. If it's very tight or won't go all the way in and you force it, it puts strain on both the teeth and the retainer.

How can I tell if my retainer wire has come loose?

If the wire feels raised or catches when you touch it with your tongue, or floss slides easily under the wire, it may have come loose. Even if just one point comes off, that tooth gradually moves, so get it checked soon.

How long do I need to wear a retainer?

Rather than having a fixed end point, think of it as wearing it for as long as you want your teeth to stay where they are. Usually you wear it for long hours at first and gradually cut back to nights only, and a fixed retainer wire is left in place long-term if there are no problems.

How long does re-treatment for a front-teeth gap take?

For a small gap involving a few front teeth, it's often resolved in around 3 to 6 months. It depends on the amount of movement and the cause, so I'll tell you after the diagnosis.
