Up to what age can adults get orthodontic treatment?
There is no fixed upper age limit. Orthodontics is a treatment in which teeth move position inside the bone, and even in adults, when bone receives force it rebuilds itself, shrinking on one side and filling in anew on the other (bone remodeling). In the clinic, people come for consultations not only in their thirties and forties but also in their sixties. Orthodontics after 40 is covered separately in Orthodontics after 40.
The goal of adult orthodontics is not just a straight row. Overlapping surfaces can't be reached by a toothbrush or floss, so they are always hard to keep clean. I don't claim that a bad bite directly causes periodontitis, but aligning the teeth increases the surfaces you can clean.
How is it different from orthodontics for teenagers?
The biggest differences are the speed of movement and the number of variables to consider. In adults, bone remodeling is slower, so teeth tend to move slowly, and many already have missing teeth, crowned teeth, or implants.
| Point to consider | Teenagers | Adults |
|---|---|---|
| Speed of movement | Relatively fast | Tends to be slow |
| Growth | Growth can be used | Can't be used, so skeletal problems have limits |
| Gums and bone | Usually healthy | Check for periodontitis and bone loss first |
| Existing restorations | Almost none | Plan around crowns, implants, and gaps from missing teeth |
Full orthodontics usually takes 1 year 6 months to 2 years 6 months, and partial orthodontics, which moves only some of the front teeth, usually takes a few months. I don't assume it takes several times longer just because you are an adult. The amount of movement, whether teeth are extracted, and the current thickness of the bone determine the duration.
Can I get orthodontics if my gums are weak or I have periodontitis?
In many cases it is possible once the inflammation is under control. But the order matters. Pushing teeth in swollen, bleeding gums can cause more bone loss, so I first calm the inflammation with gum treatment. After that, I decide in which direction and how far to move the teeth within the remaining thickness of the alveolar bone.
| Check first | If it's fine | If there's a problem |
|---|---|---|
| Gums | Force is applied after the inflammation has calmed down | If they're swollen and bleeding, movement is postponed |
| Alveolar bone | Teeth are positioned within the bone | Pushing teeth outside the bone causes gum recession |
| Bite | Teeth are moved so the upper and lower close together | Aligning only the front row twists the back teeth |
A plan that moves teeth a lot is not necessarily a good plan. Once gums recede, they do not grow back, so the range the bone can support becomes the limit of the plan.
Can I get orthodontics if I have implants or crowns?
Yes. However, implants do not move. Because they are fused directly to the bone, they don't respond to orthodontic force the way natural teeth do, so from the start we decide together which teeth to move and which positions to use as reference points. Sometimes the implant is used as an anchor point, and sometimes the area around it is left alone and only the natural teeth are moved.
If a gap from a missing tooth is left empty for a long time, the neighboring teeth tilt and the opposing tooth drops down, reducing the space for a restoration. In this case, regaining the space with orthodontics and then placing an implant or restoration lets us sort out the upper and lower bite as well.
How we do it at Wonjin Dental
- I look at the gums and alveolar bone before age. If there is inflammation, movement is postponed.
- The appliance is chosen based on your lifestyle and the difficulty of the movement. If visible appliances bother you and you can keep up the daily wearing time, we look at clear aligners (Seraphin); if you are likely to skip wearing them or the movement is complex, we look at self-ligating brackets (Clippy C).
- If only some of the front teeth are the problem, I consider partial orthodontics first.
Gums and alveolar bone matter more than age. If a movement can't be done, I tell you so. Orthodontic diagnosis is ₩100,000, and the cost of each appliance is listed in All fees.
Frequently asked questions
Is orthodontics more painful for adults?
Pain varies more between individuals than by age. After the appliance is attached or adjusted, it usually feels achy for 2 to 3 days and then eases, and in most cases it can be managed with painkillers.
Does orthodontics cause gum recession?
If a tooth is pushed far outward where the bone is thin, the gums can recede. That is why I plan within the range of the alveolar bone and also monitor the gums during treatment.
Do adults also need a retainer after orthodontics?
Yes. Moved teeth tend to go back to where they were, so adults also wear retainers for a long time. We choose among fixed, clear, and removable retainers to suit your teeth and lifestyle.
Can I get an implant during orthodontics?
Yes, with the order planned out. Once placed, an implant doesn't move, so it is usually placed after the final tooth positions are set or precisely planned.
